Human factors play an important role in marine and offshore systems. In fact, a majority of accidents in marine and offshore operations are related to human errors. The accurate assessment of Human Error Probability (HEP) under varying conditions is an essential step to prevent human error-related accidents. This chapter provides an introduction of the most commonly used methodologies to estimate HEP for marine and offshore systems. These methodologies are Success Likelihood Index Method, Human Error Assessment and Reduction Technique, and Bayesian network. Additionally, this chapter also provides a guidance for applying these methodologies in practical context to improve the safety and reliability of the marine and offshore systems.
